Microneedling is truly a workhorse treating a multitude of skin concerns. Microneedling is a proven method of improving the texture of the skin and delivers long-lasting results. It is a fantastic treatment for fine lines and wrinkles, acne scarring, sun damage, large pores, textured skin, uneven skin tone, stretch marks and so much more.
Microneedling treatment is one of the most popular treatments in my clinic. As a skin specialist, I practice what I preach and therefore ensure that I microneedle my skin every month applying the exact approach in a clinical setting that I deliver for my patients.
So, what is the hype with microneedling?
Microneedling is the only rejuvenation treatment that maximizes penetration of medically approved ingredients and cell nutrients thereby maximizing the release of growth factors associated with positive effects leading to concurrent wound healing and collagen induction (Dr lance Setterfield).
Microneedling is a powerful treatment as a stand-alone to induce collagen addressing a multitude of skin concerns, and it is fantastic for those undergoing other treatments such as Botox and dermal fillers as well as laser treatments. It functions to strengthen the skin’s integrity adding to the overall positive rejuvenating effects of the skin as well as skin health
Why is microneeding the go-to treatment that is maintaining its popularity and is constantly in demand?
Well, it is simple.
Microneedling is suitable for all skin types and treats a multitude of skin concerns
The device is an electric unit for micro-needling the skin and can deliver multiple vertical microscopic channels. The microneedles penetrate the epidermal and dermal layers of the skin.
This in turn stimulates the natural production of collagen. In addition, it creates a pathway for the introduction of powerful effective skin formulations to maximize the treatment. The microscopic channels close after an hour post-treatment.
The single-use micro-needles are made from top medical-grade stainless steel and are sterilized and packed for safety and efficiency.
The electric needling device penetrates the skin at a 90-degree angle with a rapid motion thereby, reducing the pain of epidermal penetration as experienced with the dermal rolling treatment as it may cause skin tethering. In addition, dermal rollers are uniform needle depth and therefore it is not possible to adjust the depth according to the area and skin condition treated. Furthermore, the manual pressure exerted can cause additional uncontrolled trauma.
The difference in penetration between the dermal roller and fractional pen
Medical needling triggers a wound healing response in the cells of the dermis to self-repair and regenerate.
This controlled trauma increases the natural production of collagen. The journey of natural collagen production takes approximately 4-6 weeks before the effects are visible on the surface.
Medical skin needling can restore skin concerns with similar results to laser resurfacing with little or no side effects or downtime.
What is the idea of microneedling
At the basic level is cell to cell communication, therefore the cells begin to communicate with one another to release growth factors and cytokines in the epidermis and the dermis by keratinocytes, melanocytes, and fibroblasts. These are which are signaling molecules to initiate a cascade pathway to stimulate glycosaminoglycans (GAGs) AKA Hyaluronic acid and the release of collagen and elastic, much-needed components for wound healing and rejuvenation Therefore growth factors are a vital signaling molecule for healthy skin
What is the benefit of micronedling procedure?
Create a micro-injury of the skin through controlled trauma to reset and reboot cellular function.
It normalizes melanogenesis
Triggers the wound healing process and therefore stimulates fibroblasts and cytokines to generate collagen and elastin
It upregulates woven elastin, collagen, and GAGS.
Activates platelets and promotes scarless healing.
Facilitates cross-communication between keratocytes and melanocytes leading to even distribution of pigment
Promotes delivery of skincare ingredients for better penetration.
In essence, it is a fantastic treatment with a multitude of skin benefits such as skin rejuvenation, fine lines and wrinkles, acne scarring, even skin tone, and reduction of pore size.
Did you know that microneedling treatment with mesotherapy is a fantastic treatment for hair restoration?
An effective treatment option for hair restoration
This procedure exhibits similar results to laser resurfacing yet, does not require heat and therefore respects the integrity of skin, sparing the keratinocytes, melanocytes, and fibroblasts. From being compromised This is really important especially for the melanocytes where pigment is key therefore reducing the risk of post-inflammatory hyperpigmentation. This treatment is ideal for all skin types and colours
Why is it important to ensure that your microneedling procedure is performed in a clinical environment by a regulated and licensed skin specialist?
If the skin is over traumatized, the fibroblasts will produce collagen but it will be laid in a non-uniform manner leading to fibrosis and scar tissue formation. Therefore, controlled trauma in a professional setting through microneedling leads to scarless healing therefore the collagen and elastin produced are laid down in a uniform manner.
Did you know that there are 28 different types of collagen found in the body whereas the skin contains five types of collagen?
The dermis contains type 1 and type 3, whilst the dermal-epidermal junction contains types 4,7, and 17. These respond well to microneedling.
Production of collagen does not need to be through deep needling. In fact, at 0.3mm it is possible to induce collagen.
What is the difference between cosmetic and medical needling?
Cosmetic 0.2-0.3mm | Medical 0.4+mm |
Stimulates Keratinytes to release growth factors | Triggers the wound healing cascade |
Increases cellular turnover | Targets deeper at the dermal-epidermal junction |
Enhances cosmeceutical product absorption | Stimulates collagen, elastin, and GAGS |
Strengthens the integrity of the skin | Platelet activation |
Addresses certain types of scars | |
Hair restoration |
There is a place for both cosmetic and needling in a clinical setting depending on the skin condition that is being treated.
Preparation for microneedling
It is always good to prep the skin prior to skin needling procedures and therefore skincare is vital. Such preparation includes cosmeceutical retinoids and vitamin C.
Hydroxy Acids such as glycolic acid, mandelic acids, and lactic acid which are Alpha Hydroxy Acids, and Salicylic acid which is a Beta Hydroxy Acid in the form of a cleanser
Retinoids place the skin in a hyper regenerative state thereby increasing cell division facilitating the wound healing process, repairs the DNA of the cell. For more information about the benefits of retinoids click here.
Vitamin C induces type 1 and 3 pro-collagen and is an anti-inflammatory, protects vitamin A against oxidation, increases the absorption of inorganic iron, and protects against free radical damage and UV radiation thereby sparing the skin from being damaged making it a powerful anti-oxidant. What happens during microneedling treatment?
An in-depth consultation is made to explain the treatment and to ascertain your suitability. A full medical history is taken into account. The consultation will also help to manage your expectations.
Photographs may also be taken at the initial treatment and during each treatment to monitor the progress of the treatment.
Once you are happy with the information that they have been supplied with, understood the treatment that they have chosen they will have and have ample opportunity to ask as many questions they will then agree to the treatment by signing an informed consent to commence the treatment.
How long does the treatment last?
The treatment takes approximately 60-90minutes depending on the area as that procedure may require the application of a local anesthetic that needs time to induce its effects and cleansing prior to the actual treatment.
Does the treatment hurt?
The skin is cleaned and disinfected. A topical anesthetic numbing cream is then applied to the treatment area to make the procedure more comfortable providing you have no history of allergic reaction to anesthetic products.
Once the skin is numb it is cleaned once again to remove any anesthetic cream from the treatment area. A sterile glide medium and in this case a mesotherapy cocktail is applied to the treatment area prior to passing the medical needling device. The microneedling device is passed over the skin to create micro-channels creating controlled trauma or injury to the skin. The microchannels and the injuries are tiny; they will close rapidly leaving the flushed. This is a good indicator that the treatment has been effective
What to expect after the treatment?
Immediately post-treatment, you will look as if they have moderate to severe sunburn. The skin is slightly warm with a tight sensation. These experiences are normal and usually subside within 1-3 hours. The effects diminish within 24 hours.
Slight redness may be experienced after 24 hours but in localized areas. I will discuss any post-treatment advice and skincare needs to help to soothe and protect the skin and send you a treatment plan for clinic-led skincare that is suitable for your skin to maximize your skin health. You should treat the skin gently post-treatment and re-establish your skincare routine after 72 hours.
How many treatments will I need?
Positive results can be seen approximately one week after the treatment. Some improvement is seen after a single treatment. However, a program of 4-6 sessions is recommended ideally with monthly maintenance thereafter.
When will I see the results?
It can take up to 4-8 weeks before visible signs of skin regeneration can be seen and the process will continue through the following months thereby creating a gradual and progressive improvement.
After the course of recommended treatments improvements will continue for 6-12 months.
How safe is Medical Needling?
Medical skin needling is a safe procedure with minimal risks, side effects, and downtime as it depends on stimulating the body’s own natural healing mechanism. The needles are used in the procedure are individually packed and sterile. They are designed for single-use and therefore disposable to avoid any risk of cross-contamination. However, safety depends on the knowledge and the skill of the treatment provider.
What are the side effects?
Medical skin needling is associated with low risks and side effects. The skin may look flushed for approximately 2 hours similar to that of sunburn. Pinprick marks of blood or bruising may be present. There may also be a warm and tight sensation accompanied with a slight it for about a couple of days.
Some may experience minor flaking, dryness of the skin, and scab formation.
Rarely hyperpigmentation which is the dark patches of the skin may happen. However, this is usually resolved within 4 weeks.
It is important that the practitioner is aware of any history of cold sores in order to provide effective advice regarding the appropriate medication to prevent an outbreak.
What should be done post-treatment?
Detailed aftercare information is given to you and all will ensure that you fully understand the aftercare before you leave. I will also contact you after the treatment to check on how you and your skin are doing and assure you that I am reachable and contactable should you need further information and advice.
